New student enrollment forms
Our Enrollment Process has 3 forms:
1. Student + Family Information Form 2. Credit Card Authorization 3. Select + Book your Evaluation Date
After completing each form, please press submit to proceed to the next form.
Enrollment will only occur when all three forms are submitted.
If you are interrupted, please click SAVE at the bottom of the page. You will receive an email with a link to use to complete the forms later.
